Fill Out the Family First Life Affidavit

Please email the completed affivadit to contracting@fflgoldenstate.com.

**If you do not already have E&O Insurance you can go to http://www.napa-benefits.org/ AND choose "Option B"

Once you have sent everything in Step One, we will reach out via email with a link to access the Online Contracting

When completing the online contracting follow the steps below:

First complete the section under "My Profile"

Second click "My Appt. Requests" and submit each carrier
